3 In 1999 ,The Institute of Medicine( lOM) estimated that medical errors result in 44,000 to 98,000 deaths annually it also estimated that medical errors account for as much as $29 billion annually in lost income, disability, and healthcare costs

5 Goal 1 : Correct patient identification
By using at least two patient identifiers (name , medical record no.) whenever administering treatments or carrying out any interventional procedures (never to use the patient's room number) Always check the wrist band

6 Goal #2 : Effective commuication Among Caregivers
Effective hand over between shifts Report critical results of tests and diagnostic procedures as soon as possible For verbal or telephone orders or for telephonic reporting of critical test results, verify the complete order or test result by having the person receiving the order or test result "read-back" the complete order or test result. Standardize a list of abbreviations, acronyms and symbols that are not to be used throughout the organization (ug ) (mg) , (MgSO4), (MSO4), ) IN ) (IM , IV) (q.o.d), (q. i. d) , D/C ( Discharge ,discontinue) , inderal 40 mg , inderal 140 mg

7 Goal #3 : Improve the Safety of Using Medications
Label all medications, medication containers (e.g., syringes, medicine cups, basins), or other solutions on and off the sterile field in perioperative and other procedural settings Write opening and expiry dates on multi usage vials. Separate high risk medications (look like – sound alike) 5 rights Right drug Right patient Right dose Right time Right route

9 Goal #5 : Reduce the Risk of Health Care Associated Infections
Comply with current Centers for Disease Control and Prevention (CDC) hand hygiene guidelines

10 Goal #6: Reduce the Risk of patient fall
Initial assessment of all patients admitted to hospital Reassessment of those having fall risk Following fall preventive procedures.
