Be sure to sign or the DMV will use the only date that it has. This could make it seem that you are very late.
Employment must be listed for at least three years
Employment Only for 2 years. The requirement is to check employment for three.
The employer did not date the form. The department will use the date the driver entered, which could be weeks in advance.

The regs say that the DS-873 and the DS-875 may not be done on the same day. Good catch.

Don’t forget to date the form.

Peripheral vision must be at least 70 degrees in each eye
Be careful of corrections, they must be initialed.
DS-874 Vision – Eye sight is not sufficient to pass. 20/40 or better in each eye
Horizontal Field of Vision should be marked in degrees in each eye, not just check marks

Remember that you can’t blame the doctor for filing incorrect paperwork. It is the carrier’s responsibility.
