Fatality Overview Date & Time: 11:45, June 19, 2007 (Tuesday) Place: new constructions of ○○ multiplex house site Victim: mold carpenter of a contractor (male/53) While the victim was installing a form on the lower part of excavated masonry retaining wall, old masonry and block wall collapsed and a falling stone hit the victim in the side. He was taken to the hospital. However, he died during medical treatment. (overall process rate was 3%)

on the lower part of masonry 3. Fatality Description At about 07:30, 7 workers (employer, carpenter 2, 2 iron bar workers, B/Operator) including the victim went to the site. From 08:00 to 11:40, they excavated ground on the bottom the existing masonry retaining wall (width: 0.8m, depth: 1.0m) using a small backhoe (Komatsu pc 18, so called, mini excavator). After that, they inserted iron bar (H: ≒ 2m) into the ground to install forms (euro-from). At about 11:45, the victim was installing joist (pipe for steel pipe scaffold) and sleeper (2’×2’ rectangular lumber) on the forms. At that time, block for the masonry and wall collapsed and a falling stone hit him in the side. He was took to the hospital. However, he died during medical treatment. Fatality Cause Insufficient protections against collapse such as preparatory study - For excavation close to old masonry and block wall, you must conduct a preparatory study to prepare protections against collapse.
