Figure 1 (A) A 6-year-old boy presented with abdominal discomfort and chronic anemia. His chest X-ray showing an air density lesion with an air-fluid level in the midline of the lower chest indicating a hiatal hernia (black arrow) and mild cardiomegaly due to the chronic anemia. The cardiothoracic ratio is 55.5%. (B) An upper gastrointestinal series showed a huge para-mediastinal mass (white arrow), confirming the diagnosis of a hiatal hernia.
