1 Embolization of a Perigraft Gutter Leak with Coils and N-Butyl Cyanoacrylate through the Pseudolumen Using a Triple Coaxial Catheter System after Endovascular Aortic Repair with a Chimney Graft  Akira Ikoma, MD, PhD, Motoki Nakai, MD, PhD, Morio Sato, MD, PhD, Yoshiharu Nishimura, MD, PhD, Yoshitaka Okamura, MD, PhD  Journal of Vascular and Interventional Radiology  Volume 25, Issue 10, Pages (October 2014) DOI: /j.jvir Copyright © 2014 SIR Terms and Conditions

2 Figure (a) Preprocedural angiography (left anterior oblique position, 50°) after debranching and bypass operation shows an expanded dissecting thoracic aneurysm from the aortic arch to the descending aorta. Asterisks mark the pseudolumen. LCA, left coronary artery. (b) CT angiography 6 months after debranching TEVAR with a chimney graft shows an endoleak in the pseudolumen (arrow). A chimney graft was placed in the innominate artery (arrowhead). (c) Manual angiography from the microcatheter through the pseudolumen indicates that the leak (arrow) originates from the perigraft gutter. (d) Illustration of the PGL embolization technique. A microcatheter was advanced to the pseudolumen of the descending aorta via the reentry hole (asterisk) in the abdominal aorta, and then to the true lumen through the entry of the aortic arch with use of a TCCS. The microcatheter ultimately reached the perigraft gutter, allowing embolization with coils (black arrowhead) and NBCA (white arrowhead). (e) Ascending aortography after embolization of the PGL shows the absence of a pseudolumen or an endoleak. (f) CT angiography 6 months after embolization shows no evidence of an endoleak.
