4 “Healthy in the Hills” The community of Williamson, WV is dedicated to building a culture of health through holistic community and clinical interventions in order to stimulate a thriving local economy centered around wellness for the entire population. We are looking to mentor other small, rural towns across America in building a culture of health that links economic development, entrepreneurship, and local foods through a health lens. We are interested in learning how to share the framework for building a culture of health that can be locally implemented and deeply connected to the local culture, people, and sense of place.

Implemented a Community Health Worker model to address high rates of diabetes among target population that is now being integrated into the primary care setting. Significantly decreased HbA1c levels among diabetic patients. % of patients with controlled HbA1c levels < 9% rose from 9.2% to 33.3% within 18 months. Significant reduction in # of ED visits among patients.
