2 Social Services Outreach
Our team assists potentially eligible individuals to access the benefits they need through education and application assistance. We can help to remove barriers to empower individuals to receive necessary services in the community.

3 Social Services Outreach Coordinators
As Certified Community Partners we assist potentially eligible applicants in applying for benefits such as SNAP, Medicaid, TANF, Medicare Savings, Women’s Health Program and CHIP Perinatal. Online at YourTexasBenefits.com Or by submitting a paper application

4 Social Services Outreach Coordinators
Are available to come to your site with their Wi-Fi hotspot, laptop and scanner during your hours of operation Will provide marketing materials and information on the benefits programs

5 How to Partner with CAFB’s SSO Team
Advocate – Material distribution, for organizations with limited space and limited interaction with clients. Host – Hosting site for SSO, for organizations with lots of space and that are accessible to many community members. Referral Partner – Non-Hosting referral partner, organizations with lots of one-on-one interaction with clients but that may not have enough space to host us or a location that may not be easily accessible to most clients. Ambassador – Hosting referral partner, organizations that have on site resources who can help spread the word and who have space to host as well as a fair amount of one-on-one interactions with clients.

6 Partnership Appreciation
CAFB tracks the number of individuals served by the SSO team The SSO team can provide data for partner agencies to inform them on the number of individuals served at their sites, or by referral from their site.

7 Hill Country Nazarene Helped 41 families to access $101,279 in SNAP benefits, 42,187 meals and had a local economic impact of $181,290.

8 Trinity Center Helped 123 families access $303,838 in SNAP benefits, 126,561 meals, and had a local economic impact of $543,870.

9 Fredricksburg Food Pantry
Helped 49 families access $121,041 in SNAP benefits, 50,419 meals and had a local economic impact of $216,664.

10 Total Impact for Our 21 County Service Territory
In Fiscal Year 2015 the SSO Team: Completed 1,755 SNAP applications This equates to – $4,335,246 in estimated benefits $7,760,091 going back into the local economy 1,805,806 meals to hungry families
