2 Directions Administer 1 on 1
Each assessment should be completed at the end of the month stated Provide manipulatives as needed Keep record of individual students responses and whole group responses Feel free to adapt questions

3 Question 1: Number Sense
Start at 1 and count as high as you can Stop the student if they reach 100

4 Question 2: Number Sense
Count by 10’s to 100

5 Questions 3-5: Number Sense/Geometry
Provide students with counters or any available manipulative Tell the student to show the number 7 using counters If students show incorrect number, correct him or her before continuing to the next question Tell student to write the number 7 Have student draw an equal number of triangles Repeat with the numbers 9 and 14

6 Question 6-8: Number Sense
Show the number 3 Ask students to name the number that comes before and after the number 3 Repeat for the numbers 12 and 18

7 Question 9-10: Number Sense
Using flashcards, have students order the numbers 7, 8, and 9 from least to greatest Repeat using the numbers 15, 16, and 17

8 Question 11: Number Sense
Give students 30 counters Have them make groups of ten

9 Question 12: Number Sense
What is the name of this coin? What is the value of this coin?

10 Question 13: Number Sense
Identify this coin What is the value of this set of coins?

11 Question 14: Computation
Jake has three crayons. His friend gives him three more crayons. How many crayons does he have in all?

12 Question 15: Computation
Crystal has six erasers. She gave two to her friend. How many erasers does she have left?
