2 DEMENTIA ENABLEMENT PROJECT
Introduction & Background The Life Changes Trust is an independent Scottish charity, established in April 2013 with a £50 million endowment from the Big Lottery Fund. The Life Changes Trust was created to drive real and meaningful improvement in the lives of young people with experience of being in care and people affected by dementia. The LCT vision is that these groups see a positive and permanent shift in their quality of life, well-being, empowerment and inclusion. LCT will fund organisations and partnerships which share their mission and can support the changes they want to see.

3 DEMENTIA ENABLEMENT PROJECT
Why is it needed? The majority of older people live in their own homes and this is set to increase with people living longer and a focus on caring for people in the community Whilst there has been significant research on new build design for people living with dementia there is not a vast amount of information about adapting existing stock People living with dementia can stay independent for longer at home given the correct support for themselves and their carer. People living with dementia are more likely to have falls or accidents in the home and therefore it is especially important to ensure that the home is safe and free from potential hazards at an early stage.

Why Care & Repair? Care & Repair services have been assisting people to remain living independently within their own homes for over 30 years They have existing relationships working with adaptations and telecare and facilitating hospital discharge Preventative work / home safety are a priority to Care and Repair Teams work across multi-disciplinary and departmental boundaries with Housing, Health & Social Care, Police, Fire and Rescue and voluntary organisations Localities Aberdeen – urban Angus – mix of rural and urban Lochaber & Lochalsh & Skye – remote rural

5 DEMENTIA ENABLEMENT PROJECT
What is it? 3 areas in Scotland working with people who are living at home with dementia to improve their daily living by providing improvements, adaptations or alterations to their home. Dementia trained, dedicated and skilled staff to discuss and agree works with the individual (and carer) which will enhance their life in their home and then carry them out. An early safety check to advise about any immediate changes and then an open opportunity to come back for additional works when needed at the individuals pace. Close working with existing services such as Dementia Link workers, Telecare, Social Work and Health and Carers organisations to promote a joined up approach.

Outcomes People living with dementia and/or their carer(s) can contribute to designing a service which is provided in a timely, respectful and mutually agreed manner and suit the recipients needs, which helps retain independence, safety, security and activity in the home and the community. Existing services will be informed and new services developed for design, adaptations and equipment to suit people living with dementia that can then be adopted by others and incorporated into future dementia provision for individuals and which can influence needs assessments in the future.  The service evaluation may show links between works needed and the type of dementia or the stage at which enablement is offered.

Case study - Angus Gentleman diagnosed with vascular dementia and referred by post diagnostic dementia team. Becoming more unstable on his feet and extremely frustrated about his increasing deterioration and inability to contribute to daily living as he previously did. Losing confidence in using his computer as he cannot remember passwords. Demonstrated how to use fingerprint recognition for on-line banking instead of passwords and couple immediately bought a tablet to enable him to do this. Now he is able to do the shopping and banking again with confidence. Work done:- Thumb turn locks to front and back doors, privacy lock to bathroom. Grab rails at both doors externally. Telecare smoke and CO detectors fitted. External PIR lights installed from and back doors for safety. Digital clock and weekly planner given. Daylight bulbs and night light fitted.

Case study – Aberdeen Couple aged 87 & 91 both living with dementia. Family contacted service and met at property. Mr who has vascular dementia and was hard of hearing was not communicating with anyone. Mrs is totally unaware of what is going on around her. In order to communicate with Mr the underside of the whiteboard was used and written on to allow him to point & communicate. His daughter will continue to use this method. Enablement check carried out and a number of issues identified. Equipment - Day Planner/Clock/Nightlights/Day light bulbs/2 sink plugs Disconnect gas cooker – couple were leaving it on Attendance Allowance for both Referral for bathroom to be adapted to their needs (OT) External handrails at back door to access garden through project Referral for Telecare

18 DEMENTIA ENABLEMENT PROJECT
Case study - Lochaber Client affected by Vascular Dementia and living on his own with no family or friends in the area. No visitors or neighbours and his sister lives in Plymouth. Home Safety Enablement check carried out and the following interventions carried out:- 3 different kinds of clock supplied 5 daylight bulbs fitted lock cover fitted over fridge and cooker sockets taps repaired blocked sink cleared specialist remote control (TV) supplied external grab rails supplied and fitted talking movement sensor at front door reminding him to check the time referral to Memory nurse re attendance at local Men’s Shed

19 DEMENTIA ENABLEMENT PROJECT
Case study – Lochalsh & Skye The client was known to the Handypersons Service as she has had several tasks carried out by them. The lady had been a keen gardener having won the Housing Association’s Garden competition in Unfortunately her deteriorating condition has meant that she is no longer able to look after her garden to the same extent. Interventions included: Bushes on the driveway and around the house which were overgrown and obstructing her movement were cut back to enable her to move around the garden safely. The garden gate was not shutting properly so broken hinges were repaired. Daylight bulbs were supplied in the hall area. The broken leg on her foot stool was repaired.
