Process Objectives Subjective Objectives There are different ways to measure things. We are going to talk about measuring 3 different kinds of objective.

5 You Need All Three! Process Objectives: “How many of these did we do?”
Subjective Objectives “Do they like us?” Outcome Objectives: “Did the patient get better?” Why do you need all three? If we see a lot of patients, but none ever get better, what are we doing? Or if they are getting better, but we’re only about to see 1 patient a day, is that a good outcome? Or if they are getting better, but no one wants to come to us because we treat them badly, then what? You Need All Three!

Use them in staff meetings to look at trends and improvement (or not) Share them with your board Use them to set organization wide goals Use them to reinforce standards among staff Use them in online newsletter Use in annual report Use in foundation grant applications and reports Publicize at your annual gala Use them to craft a story about your clinic to the media

27 So What Do WE Want to Do? Start data collection process with all member clinics Not currently looking at ‘standards’ per se Launched last year, piloted this year What We are Tracking: Diabetics – A1C <7% and A1C >9% Any patient with a diagnosis of diabetes, regardless of co-occurring diagnoses Hypertensives – BP <140/90 Any patient with a diagnosis of hypertension, regardless of co-occurring diagnoses

28 Definitions and procedure
Patient definition Eligible clients are those who: Have been with the Clinic at least two years Have had at least 2 medical appointments Last appointment was within the last 6 months Procedure If you can easily pull up data from using all of your patients, great. Otherwise, you can do a sample. Sampling Procedure (if you need to use this rather than your whole database): Do a 10% sample of their patients that have the above diagnosis. For example, if you have 320 patients with diabetes, you will need 32 results. If you can pull the data up on ALL your patients, that’s fine.

30 Timelines Timeline Twice a year, each clinic will collect data – once in January and once in July, with results due to the IAFCC by the 30th of the following month. Timeframe: For January due date, look only at patients who have had a visit in the previous six months , ie, July 1- Dec 31. For the July due date, look only at patients who have had a visit in the previous six months, ie, Jan 1 – June 30.
