Important Information Benefit Offerings for Highlights How To Enroll Thank you for coming to learn about Newsday’s Benefits Program and Open Enrollment for 2016 Today we’ll review important highlights and how to enroll. Open Enrollment is from November 19th through December 3rd. Annual Open Enrollment for 2016 Benefits starts at 6:00 am EST on Thursday, November 19, and ends at 8:00 pm EST on Thursday, December 3

2016 Open Enrollment Learn about your benefits and what’s important for 2016 Read the 2016 Benefits Guide on MyHRInfo (via Marquee or at myHRInfo.Cablevision.com) Attend an Employee Meeting to find out how to get the most out of your benefits. Decide which options are right for you and your family Choose among a variety of plans and options Verify that any covered dependents are eligible Review the information for your beneficiaries to be sure it is up to date and accurate. Go Go online to MyHRInfo (via Marquee or at myHRInfo.Cablevision.com) to enroll During the Open Enrollment season, you have three action items: First – LEARN about about your benefits and what’s important for 2016 by reading the 2016 (two-thousand sixteen) Benefits Guide on MyHRInfo (via Marquee or at myHRInfo.Cablevision.com) and by attending an Employee Meeting Second – DECIDE which plan options are right for you and your family. Some plans offer different levels of coverage, so you’ll need to choose what’s best for you. Other decisions include whether your covered dependents are still eligible and if all the information on your beneficiaries is up to date and accurate. Finally – GO to HyHRInfo on Marquee or at myHRInfo.Cablevision.com to enroll We’ll go into further detail on how to enroll later in the presentation. ---

2016 Annual Open Enrollment
The 2016 Annual Open Enrollment period for your Health and Welfare benefits begins 6:00 am EST on Thursday, November 19, and ends at 8:00 pm EST on Thursday, December 3 If you do not enroll during Open Enrollment, your 2015 benefit elections will continue in 2016 with the exception of the Health Savings Account (HSA) and the Flexible Spending Accounts (FSA). If you would like to enroll in the HSA or an FSA in 2016, you must actively submit this election during Annual Open Enrollment You should log on to MyHRInfo during Annual Open Enrollment to make sure you have the coverage you intend for 2016,and that any dependents currently enrolled remain eligible for coverage in 2016 Again, the 2016 Annual Open Enrollment period starts at 6:00 am EST on Thursday, November 19th and ends at 8:00 pm EST on Thursday, December 3rd. This is your opportunity to make your Medical, Dental, Vision and other benefit plan elections for the new year. If you do not enroll during Open Enrollment, your 2015 benefit elections will continue in 2016 with the exception of the Health Savings Account (HSA) and the Flexible Spending Accounts (FSA). If you would like to enroll in the HSA or an FSA in 2016, you must actively submit this election during Annual Open Enrollment

Dependent Verification
Eligible dependents are defined as: Your spouse (provided you are not legally separated or divorced) Your same- or opposite-sex domestic partner Your unmarried child(ren) or those of your spouse/domestic partner, until December 31st of the year in which they reach age 19 (or 24 if they are full-time students) for Dental, Vision, Life and AD&D coverage. For the Medical Plan, you may cover your or your spouse/domestic partners’ children up to the last day of the month in which they reach age 26 Your unmarried mentally or physically disabled child(ren), or those of your spouse/domestic partner, regardless of age, who have been classified as disabled and incapable of self-support because of their disability, provided they became disabled while covered under the Plan If someone is no longer eligible be sure to remove them It’s important this year to review and update your eligible dependents when you enroll in your benefits. If a dependent does not meet our dependent eligibility guidelines, you need to remove them from coverage. If you newly enroll a dependent, you will receive a packet from Xerox HR Solutions that you will need to respond to in order to verify your dependent’s eligibility. Our goal with this dependent verification process is to be able to continue providing a comprehensive and affordable benefits package and to keep our commitment to act only in the interests of plan participants and their eligible dependents.

Beneficiaries Open Enrollment is also a good time to review the information on file for your beneficiaries Beneficiary information should be up-to-date and complete Please review your Life Insurance and AD&D beneficiary(ies) on MyHRInfo and update their information, if necessary Please review your Retirement Plan beneficiary(ies) in Fidelity’s system – netbenefits.com You should confirm that the information on file for your beneficiaries is up-to-date and complete.

Health Care Costs While implementation of the Affordable Care Act is contributing to rising health care costs, Newsday is not passing those costs on to you. And, thanks to your ongoing efforts to use your Newsday benefits wisely and to take advantage of wellness programs that promote healthy lifestyles, your costs for 2016 benefits will remain at last year’s levels. Wellness360 The Medical Plan covers a variety of preventive care services at no cost to you Wellness360 (available through Marquee or at Wellness360.Cablevision.com) provides access to tools and resources dedicated to your physical wellness such as health screenings, health assessments, wellness coaching and more In addition, UnitedHealthcare offers resources at MyUHC.com to support you if you enroll in a Newsday Medical Plan. For example: Virtual Visits lets you see and talk to a doctor from your mobile device or computer without an appointment Personalized Health Support -- You have access to a team of dedicated nurses, assigned to work specifically with Cablevision employees and their family members. If you are facing a long-term chronic illness or a complex health issue, a nurse will help you manage your condition by providing information, connecting you with resources and giving you tips to work more effectively with your doctor Health4Me is an app that makes managing health care a lot simpler. You can easily access your health plan ID card, benefit amounts, account balances, and Personal Health Records -- even pay a doctor's bill and much more Newsday is committed to providing an environment where you and your family are supported through a competitive and robust benefits program. In addition, Wellness360 offers valuable tools and resources like health assessments and screenings to help you maintain a healthy lifestyle. Wellness360 is available to all Newsday employees. Because preventive care is available at no cost to you, that’s one less hurdle to stop you from taking advantage of these services. Preventive care includes an annual physical examination for yourself and your eligible dependents, but it’s much more than that. It can also include: Well-child care, including immunizations, Adult immunizations and screenings, including for blood pressure, diabetes and cardiovascular disease, Health services for women, including well woman exams, HPV DNA testing, and breastfeeding and post-partum counseling, as well as birth control devices that include Tier 1 contraceptives, and Adult counseling on tobacco and alcohol use, physical activity and depression. And if you are covered under a Newsday Medical Plan, you can access even more resources through UHC. Use the resources available to you to make this your healthiest year ever. Newsday Prescription Drug Coverage
Prescription copays will remain the same in 2016, and will continue to apply towards the deductible. Three-Tier plan – Generic, Preferred Brands and Non-Preferred Brands This year, if a generic is available and you choose the brand name, you will pay the generic copay PLUS the difference in cost between the brand name and the generic Specialty Designated Pharmacy – if you are taking certain high-cost brand medications, you will be contacted and encouraged to use the mail-order pharmacy or switch to a lower cost medication for continuation of network benefits Quantity limits – quantities for certain prescriptions will be limited to protect your safety and minimize waste Read slide aloud

The amount you pay for in-network eligible expenses only applies towards your in-network deductible and out-of-pocket maximum ** Out-of-Pocket Maximum includes the deductible There are no changes to your costs for medical and prescription drugs in 2016. But remember – if you choose a brand name when a generic is available, you will pay the generic copay PLUS the difference in cost between the brand name and the generic. And if you use certain, high-cost specialty drugs, you will need to use the mail order pharmacy or switch to a lower-cost drug.

Health Savings Account (HSA)
If you enroll in the Newsday Medical Plan, you may be eligible for the Newsday HSA You may use the money in your HSA to pay for eligible expenses that you and your eligible dependents incur The 2016 IRS limit (includes both the Employee and Employer contribution) is: $3,350 for Employee Only coverage $6,750 for Employee+1 or Family coverage If you participate in the Newsday Medical Plan, and are eligible for an HSA, the Company will continue to contribute to your HSA in 2016 as follows: Employee Only coverage - $500 Employee+1 or Family coverage - $1,000 If you do not choose to make an employee contribution toward your HSA during Open Enrollment, or when you initially enroll in medical coverage, you will not have the opportunity to newly elect a pre-tax employee contribution during the year Read slide aloud You must actively enroll in the HSA for 2016 during Annual Open Enrollment. Your current HSA election will not roll over into 2016, even if you elected to contribute to an HSA in 2015.

Health Savings Account (HSA)
The Company will contribute to a Health Care Flexible Spending Account (FSA) if you participate in the Newsday Medical Plan or another high-deductible health plan (HDHP) but are not eligible for the HSA because: you are also covered by Medicare* or receive benefits under TRICARE or the Veterans Administration you are also covered by a non-HDHP health plan, such as a spouse’s plan you are claimed as a dependent on another person’s tax return you are covered by a general purpose health care FSA or health reimbursement account (HRA) Ii applicable, the Company will contribute to a Health Care FSA as follows: Employee Only coverage - $500 Employee+1 or Family coverage - $1,000 *Those eligible for Medicare will receive a separate communication and must complete a verification form no later than Thursday, December 3 Read slide aloud

Health Savings Account (HSA)
To enroll in a Health Savings Account: Simply go to MyUHC.com – in the “Learn More About” section, click on “Health Savings Account” – click on “open their HSA” When entering the Account Holder information: Employer: newsday Employer number: Read slide aloud You must actively enroll in the HSA for 2016 during Annual Open Enrollment. Your current HSA election will not roll over into 2016, even if you elected to contribute to an HSA in 2015.

Estimate your annual out-of-pocket expenses for 2016 to make your Health Savings Account or Flexible Spending elections Verify that any covered dependents are eligible Review the information for your beneficiaries to be sure it is up to date and accurate. Ready to Enroll? Log on to MyHRInfo at After you have made all of your 2016 elections, review the Health and Welfare Enrollment page that will show the coverage you will have in 2016 and your per-pay-period contributions for each option elected Review your elections to ensure that they are accurately recorded. If correct, click Submit to save and record your elections Review your Health and Welfare Confirmation page Take this opportunity to review the dependents you have enrolled for 2016 to make sure they meet the eligibility requirements. Print a copy of this page for your records You will not receive a confirmation statement in the mail and will not have another opportunity to enroll for 2016 once the Annual Open Enrollment period ends, unless you experience a Qualifying Life Event during the year and make changes within 31 days of the event If you do not see a Health and Welfare Confirmation Page, you did not submit your elections correctly. Return to your elections and submit again. You can access your enrollment via MyHRInfo as many times as you would like during the Annual Open Enrollment period To enroll, simply log on to MyHRInfo using your User ID and password. Your User ID is your six-digit PeopleSoft ID, also known as your “Employee ID” number. You’ll need an iAuthenticate password to access the system. If you have a Network ID, or login for Newsday’s internal network, your iAuthenticate password is the same as your Network password. If you do not have a Network password, you will need to register by clicking on the “Register with iAuthenticate” link. Then you’ll be ready to enroll. Once you have finished your review and made your elections for 2016, you must ensure that these choices have been finalized. When you’re ready to record your elections, click Submit so that the elections are stored and valid for 2016. Remember, if you do not click Submit, your elections will not be activated. Once your 2016 elections have been submitted, you will be taken to your Health and Welfare Confirmation Page. Review your 2016 coverage and covered dependents and when all information is accurate, print a copy of this page for your records. You will not receive a confirmation statement in the mail and will not have another opportunity to enroll for 2016 once the enrollment period has ended. If you need to make changes to your elections before the enrollment period closes, simply access MyHRInfo, 7 days a week, starting Thursday, November 20th, through Thursday, December 4th. You can access your enrollment via MyHRInfo as many times as you would like during Annual Open Enrollment. Remember, your elections on file as of Thursday, December 3rd at 8:00 pm EST take effect on January 1, 2016 and remain in effect until December 31, 2016, unless you experience a qualifying life event and make any changes within 31 days of the event. Please refer to the 2016 Benefits Guide for a list of Qualifying Life Events. Please keep a copy of your confirmation for your records. You may be asked to provide a copy if there is a problem with your enrollment. ---
