3 CLCH services summary We have 78 separate CQC registered services
Specialist services long term condition management for people living with diabetes, heart failure, Parkinson’s and lung disease homeless health services community dental services sexual health and contraceptive services psychological therapies Adult community nursing Including 24 hour district nursing, community matrons and case management. Children and family services Including health visiting, school nursing, community nursing, speech and language therapy, blood disorders and occupational therapy. Rehabilitation and therapies Including physiotherapy, occupational therapy, foot care, speech and language therapy, and osteopathy. Long-term condition management Supporting people with complex and substantial ongoing health needs caused by disability or chronic illness. Walk-in and urgent care centres Providing care for people with minor illnesses and injuries plus providing a range of health advice and information. End of life care Supporting people to make decisions and to receive care at the end of their life.

4 Five year strategy We want to secure our status as a community healthcare provider and establish ourselves as a high quality, great value provider. We want to be recognised by patients, staff, commissioners and partners across health and social care provision as a best in class NHS organisation. A Trust that patients acknowledge as delivering great care. A Trust that attracts and retains the very best staff. A Trust that commissioners and partners want to work with to realise the fundamental challenges facing health and social care in the years ahead. We want to embody the very best of the NHS by offering compassionate care to all. Helping children get the best start for a healthy, productive, fulfilling life. Helping adults recover quickly from illness and injury and retain their independence during long-term illness. Helping to protect and support the most vulnerable in society. And as people reach the end of their life we want to deliver the very best care that allows them to die with dignity and respect in the place of their choice. We want to build a highly engaged workforce who are motivated to provide great care every day to everyone; led by clinicians with devolved power to improve patient care through innovation and transformation. To allow us to achieve our ambitions we want to secure a healthy level of growth within the M25 and its close environs, using that growth to offer stability, quality and value to our patients and commissioners.
