History of Assisted Living
Motivated by distaste of nursing home care, a shift occurred in senior housing beginning in 1979 to The shift initiated the service capacity and consumer-centered care model. In 1986 to 1993, the providers, consumers, and government became interested and created four distinguishable kinds of assisted living communities. Then in 1994 to 2000 Wall Street brought financial investors, and quality of care became the ideal model. Today, due to older adults living longer, eruption of chronic illnesses, and the exploding boomer market, assisted living is reorganizing and examining its standards and how it provides care. Current Status AND THE NEED FOR MORE REGULATIONS
Assisted living expanded largely through the private or out-of-pocket pay market. Although as of July 2013, close to forty-five states provide some public funding under Medicaid waivers. Because assisted living was initially a private pay community, regulatory oversight lagged behind development. GOAL: To enable low-income, Medi-Cal eligible seniors and persons with disabilities, who would otherwise require nursing facility services, to remain in or relocate to a community setting in a Residential Care Facility for the Elderly (RCFE) or public subsidized housing. The program was converted into a five-year federal waiver program as of March 1, Pending final approval, the waiver program will be renewed through February 28, 2019. Eligibility: Participants require a nursing facility level of care since ALW is designed to serve people who would otherwise need nursing home care Contracted Care Coordination Agencies use a standardized assessment tool to determine the clients need for nursing home level of care Must be age 21 and older. Care Planning: Participating RCFEs must develop a care plan to implement the service plan for each resident. A licensed, Medi-Cal certified home health agency will implement care plans for participants who live at public housing sites. In this setting, the services provided are called Assisted Care. Choosing a Facility Participants select the facility or provider of their choice. Care coordination agencies will inform participants about available facilities and providers. Given that assisted living residents take an average of 10 prescription medications, let’s do the some math to see how many medications may get passed to residents every month. It is not unusual for the person responsible for passing medications in a larger Community to pass medications to 35 residents. 35 residents X 10 medications each=350 a day 350 meds per day X 30 days per month=10,500 medications passed each month! Pills are difficult to swallow. Request a liquid if available. See guidelines for crushing medications. The side effects are too disruptive or severe. Consult MD for a different medication or different dosing schedule. Manipulative behaviors, this may be the resident’s way of gaining power. Allow greater resident control in other areas The resident is agitated or confused. Re-attempt several minutes later, do not draw attention to the issue. Remove environmental distractions, such as noise or other residents. The resident was not informed of changes to his/her medication regimen. MEDICATION ORDERS Types of Orders Physicians give many different types of orders Regarding a resident’s medications. They may include: Routine Order are given (i.e., the same time every day). Routine orders are carried out until the physician cancels it by another order or until a prescribed number of days elapse. PRN Order are not given on a set schedule; they are given “as needed.” In addition to the components of a physician order above, a PRN order will include maximum dose in a 24 hour period and symptoms which would warrant the use of medications. Hold Order Sometimes a physician will want to hold a medication for a period of time. An example of this may be a resident who is scheduled to have surgery. The physician (or surgeon) may want to “hold” certain medications for the day(s) leading up to the surgery. Discontinue Order Sometimes called a “D/C” order, this is an order from the physician to stop a medication. HOSPICE ONLY for residents on hospice, a relative or
friend NOT receiving monetary or any other form of compensation for their services, and who is trained by the hospice agency may Administer medications to his/her relative or friend if the following is done: It is specified in the hospice care plan The hospice agency provides a statement for the licensee’s records that the relative or friend has been trained There is a plan in place to ensure that the resident can receive the needed medication by a licensed health professional if the relative or friend fails to arrive at the appointed time.
