The statement includes measurable goals for each area, defines the patient population and specifies the team members. Scenario: The Emerald City Linkage to Care Project
In the Emerald City EMA, 7,000 PLWHA are served by the Ryan White system, including a Part C with 2 clinic sites, 3 other medical sites that get Part A and B support, A Part D that serves Green youth, and 2 other agencies with Part A funding that provide case management. The Oz AETC provides training for the region, including providers at Kaiser Oz. A cross-Parts QM Committee for the City has formed and gets regular performance reports from all the Ryan White providers. The ECQMC notes worrisome performance on the HAB retention in medical care indicator; only 74% across all agencies. At a meeting facilitated by chairman C. Lion, the Part A grantee, the Committee decides to organize all the Parts behind a joint QI project on this topic. A project team headed by W.W. West (the Part B rep) is delegated to further refine and develop this idea into a charter for the full Committee to consider.

The Quality Management Committee
Builds the HIV program’s capacity and capability for quality improvement Involves program leaders and other key staff to cement their personal commitment to quality Feeds and focuses the quality work, in order to make it sustainable and effective In a large organization, links the HIV quality program with the organization’s overall quality program Let’s begin with the quality management committee. As we mentioned in our introduction, the quality management committee leads the HIV program’s quality improvement activities. Its job is to build the program’s capacity and capability for improvement. The committee should include leaders from different areas of your HIV program and their involvement in the committee’s work will help strengthen their personal commitment to quality as well. If your HIV program is part of a larger organization, you will want to coordinate your quality improvement work with that of your organization as a whole. The quality management committee helps to do this.

This slide shows examples of committee membership for two different types of HIV programs: one at a major teaching hospital, the second in a neighborhood health center. The bottom line is, the people who lead the different parts of your HIV program should be on the quality management committee. These lists also indicate that not all staff are members of the quality management committee, although all internal professional groups should be represented. Remember that most if not all staff should be involved in key quality improvement activities.

The overall responsibility for the HIV/AIDS Center lies with the Medical Director who authorizes the quality committee to plan, assess, measure, and implement performance improvements throughout the entire HIV/AIDS Center. Getting Committee Work Done
Identify a chair for the committee Set meeting frequency and duration Document your progress Establish communication channels Train committee members on quality improvement To be effective, the HIV quality committee needs to have basic operational ground rules in place: - One individual needs to be selected to direct the committee activities and report to other parts of the HIV facility. - Regular quality committee meetings should be set up at least bi-monthly. The scheduled time should be as convenient as possible for all committee members. - Every quality committee meeting should have an agenda to guide points of discussion. If you do not have an agenda, use the last few minutes of the previous meeting to create one. Likewise, minutes of the meeting should be recorded to summarize the discussion points and remind committee members about action items to be performed. - It is important to keep staff and consumers informed about committee activities, even if they are not on the committee. Many quality committees do this effectively by sharing meeting minutes with staff and consumers, and mentioning progress at routine staff meetings. - To build capacity of current and future committee members, routinely educate all about quality improvement methodologies and tools.

Facilitating Innovation and Change
Removes barriers to making and sustaining improvements Prepares staff for change Promotes communication: gives everyone at the facility a voice in the quality management program As its second responsibility, a quality committee removes barriers to achieving and sustaining improvements. This may sometimes require changing policies. One example of such a policy change considered by some HIV programs is whether to authorize nurses to read PPDs, as a way of increasing the number of PPDs placed and read. Some level of training may be necessary to prepare staff for change. In all cases, promoting open, two-way communication to ensure everyone at a facility feels he or she has a stake in quality improvement is an important role for the quality management committee.

Providing Guidance and Reassurance
Oversees the progress of quality activities Helps quality improvement teams in their work Supports changes that result from quality improvement projects Listens, observes, responds to staff concerns Third, the quality management committee guides the progress of quality-related activities to ensure that quality improvement teams are staying on track. Quality improvement projects will involve some measure of changing the status quo. Departures from routine conventions can be challenging. The quality committee needs to listen, observe, be responsive to staff needs during the improvement process, and provide support and encouragement.

Allocating Resources Makes staff time available for quality committee meetings and quality improvement project team work Ensures that staff has the tools, knowledge and data necessary to participate in quality improvement work HIV facilities already face many time constraints and resource pressures on a daily basis. The HIV quality committee needs to ensure that staff has the time, tools, knowledge and data to do quality improvement work. The quality management committee needs to make sure that these elements are available.

Establishing a Common Culture
Demonstrates a true commitment to the quality program Successful buy-in to the quality program means “not to get people to do what they are told but to do what they are not told” Finally, as we have mentioned, improving quality requires change, and change is difficult. Making decisions based on data, not blaming individuals, being open to new ways of doing work - these are all elements of the new culture the quality management program is trying to build. The members of the quality management committee speak with a constant and consistent voice to encourage this new culture. Successful HIV care improvement is achieved through an organization’s commitment to, and importance they place on, quality.
