Evidence suggests that engaging parents in active partnerships: Reduces errors Improves compliance Leads to fewer no shows Encourages better self-care Reduces repeat procedures Fosters better care coordination Builds greater trust Enhances communication By definition, promoting well-being requires active engagement of families and children in developing healthy habits & skills!

7 Importance of engaging families as partners in their child’s care
Well child visits present an essential leverage point for: Engaging parents in the health and well being of their child Early identification of physical, social, emotional and behavioral issues Provision of critical anticipatory guidance and education for parents Promoting positive child and family health, resilience, social and emotional skills Gaps exist for 9 out of 10 children, whose parents report not receiving at least one of a core set of recommended services for psychosocial, developmental and other screening services, anticipatory guidance, or parental education Most patient engagement tools are focused on adults and chronic illness. To fill the gap for children, tools are needed to engage parents in identifying priorities for their child’s health and promoting open discussion of child and family relationships & issues

9 What is the Well Visit Planner?
Online parent-completed tool which takes 10 minutes or less to complete before well child visit Anchored to Bright Futures Guidelines for children ages 4 months - 6 years. Provides subjective and objective parent, family and child assessments Guides and identifies parents’ informational needs and priorities. Educates and empowers parent about child health and development Creates a personalized guide to focus well visits on family needs and priorities: Fast track focus of conversations on needs and priorities Enable targeted use of educational materials and resources Ensure priorities, needs and follow up related to health goals occurs. Available in English and Spanish Optimized for mobile use

29 Help Me Grow WVP Request for Applications
The purpose for this Request for Applications (RFA) is to solicit interest from select Help Me Grow (HMG) affiliates with an interest in promoting parent engagement in child health and supporting the efficacy of child health services in promoting children’s healthy development through the incorporation of the Well-Visit Planner (WVP) The approach for this project will leverage HMG telephone care coordinators’ access to families and the interconnection between the HMG Centralized Access Point and local pediatric primary care providers. Participating affiliates will receive technical assistance and support from the HMG National Center and the Child and Adolescent Health Measurement Initiative (CAHMI) to ensure successful implementation of the WVP.

30 Eligibility for this RFA
HMG affiliates that have achieved at least Installation in the Centralized Access Point as part of the HMG National Fidelity Assessment, and: HMG affiliates maintaining an operational HMG call line specialized to serve young children and their families Telephone care coordinators within the centralized access point have the ability to incorporate this additional resource offering to callers HMG call line has been active for at least one year and should receive sufficient calls to reach the target of WVP administrations over the project period HMG affiliates that have achieved at least Installation in Health Care Provider Outreach as part of the HMG National Fidelity Assessment, and; Conduct targeted outreach to child health care providers through training, office- based education, or workshops OR have existing relationships with pediatric practices that could be leveraged for this project

31 Participation in Project
Project Objectives: Create a Community of Practice among selected HMG affiliates to support and facilitate the incorporation of the WVP as strategy to strengthen child health practices and increase parental engagement. Identify best practices for WVP implementation and utilization to support broader spread and scale across the HMG affiliate network.

Project Activities Training and Pre-Implementation Planning WVP training provided by CAHMI: education on the tool and a review of the implementation tool kit Completion of the WVP Implementation Map that will guide staff through the planning and pilot launch Implementation Training staff of the centralized access point regarding administration of the WVP Offering the age-appropriate WVP to parents All sites will be expected to engage parents in completing the WVP by the end of the project period Addition of the WVP into ongoing education and outreach to child health providers Monitoring Progress Sites will document their experience on key implementation WVP metrics through the WVP Family Participation Tracker and through a HMG-specific portal Sites will participate in collaborative calls and project listserv to support peer-to-peer learning and discuss successful strategies across the Community of Practice
