11 HIIN Readmissions Challenge
Goal: Capture patient/family perspectives on readmission contributing factors Process: Interview up to five readmitted patients and complete a short survey by August 31 Optional interview tools and the survey link will be ed to HIIN main contacts and Readmission contacts this week
