Directorate Oil North Location Yibal Dept ONE What happened After negotiating a left turn near Yibal “C” station, a Canter driver lost control of the vehicle rolling over and landing on its roof. Directorate Oil North Location Lekhwair Dept ONE Pipe direction of movement when flexed What happened While fitting a flow line the mechanical team were placing 6” pipe on two tripods to support the pipe. At this point the mechanical foreman was walking past the pipeline at the same time as the pipeline flexed, falling from the supporting tripods landing on his right leg resulting in a fracture to his right leg. Directorate Well Engineering Location Ufuq field, Lekhwair Dept UWBW What happened While the HDU (Hose Deployment Unit) crew was laying hoses from a slow moving HDU trailer, the hose got stuck at the hose reel. As the trailer kept moving forward, the hose released from the reel hitting the technician causing him to fall landing on his right arm resulting in a fracture. Directorate Well Engineering Location Rig 31 Saih Rawl Dept UWOD What happened A forklift was lifting up the 6” line to connect to the Shale Shaker cuttings box whilst the roustabout was hand tightening the hammer union connection.  While doing so, the Roustabout encountered difficulties connecting the line and started to push it with his thigh causing the line to move towards the cutting box crushing his finger against the edge. Directorate OND Location Qarn Alam Dept ONE What happened A canter was travelling from Adam to Qarn Alam when the driver decided to turn left from the main road. A 3rd party truck travelling behind him collided with the left side of the canter resulting in damage to the vehicle and a fracture to the drivers left collar bone. Directorate OSD Location Harweel Dept OSE What happened Whilst preparing chapatti mix a cook noticed some of the mix sticking to the side walls of the hopper and decided to pushed it down with his right hand without switching the machine off. Whilst doing this his right thumb and index finger became entangled between the rotating blade and hopper body amputating both his finger and thumb. Directorate UWD Location Nimr Dept UWN What happened While installing the BOP which was joined to winch, three group part were holding the BOP to fix it on the X-tree as two individuals pushing, IP was pulling. His right hand got slipped from the BOP, gone into the gap and got pinched by the stud causing a fracture on his ring finger. Mr. Musleh asks the questions of can it happen to you? Directorate UPD Location Muscat Dept UPR What happened While carrying an empty steel cylinder (25kg) up a staircase, a Lab Engineer slipped losing his balance landing badly on the stairs resulting in multiple cut wounds. He was taken to a local clinic for first aid treatment before being referred to Khoula Hospital where an X-ray revealed a fracture to his left hand ring finger.
