An ability to see all care – everywhere – safe handovers / prescribing Understanding how patients really use our services / population health Baseline health data – for instance : how many people have Chronic Kidney Disease in NI ? (2 yr’s of my life) Missing opportunities to avoid admission, allowing elderly to remain at home, keeping population healthy and managing ill patients safely at home Enabling new ways of caring and managing disease

Encompass is a vehicle for delivery of transformational change –but to achieve maximum benefit it requires whole system change We need to deliver better outcomes Outcomes for people - Outcomes we can measure We need patients involved in everything we do / portal We need to starting acting – life in HSCNI 2021 will be different Business case delivered to DoH next week!

14 What we are delivering in Islington
Across Health and Social Care, covering GP’s, Acute, Community, Mental Health, Ambulance, Adult & Child Social Care for over 10,000 care providers Full Integrated Digital Care Record, (IDCR) Avoids the quality, duplication and timeliness issues associated with data repositories and enables care plans and care pathways to adapt organically as new information sources are added over time Federated & Real time information sharing Allows Islington to holistically mange the complex care needs of their population at an individual level and across the entire care community Care Coordination tools A true transactional capability, allowing the citizen to have access & share IDCR, to manage their own health & wellbeing, book appointments, request repeat prescriptions and be a full part in their own health care. Available via portal & app Person Held Record across 216,000 Islington citizens Initially to support Multi Disciplinary Teams, (MDT’s) and minority groups such as deaf citizens and those with mobility issues, but being considered for use as GP overflow to address challenges of access and 7 day working. Secure Video conferencing To identify problem cohorts then embedding anticipatory Care Plans specific to individual across MDT Predictive Analytics Fully embedded in solution, used at the front end to cover risk stratification and cohort selection, but also to provide a bench mark and evidence mechanism to underpin Payment by Outcomes aspirations Population analytics
