11 CAD Coronary Artery Disease narrowing of the coronary arteries that prevents good blood supply to the heart muscle. Usually caused by atherosclerosis.

12 CF Cystic fibrosis Genetic. Affecting the mucus lining of the lungs, leading to breathing problems and other difficulties

15 VD Venereal Disease also known as sexually transmitted diseases. diseases or infections that are transmitted by means of sexual contact

17 TWE @ HS Review Tap water enema At bedtime

18 ASA ii Q4H prn for HA Review Aspirin Every 4 hours As needed

19 Pt DOA. Notify coroner ASAP. Review Patient Dead on arrival As soon as possible

20 Dx: CAD, Ca lung, FUO Review Diagnosis Coronary artery disease Cancer Fever of unknown origin

21 Severe pain RLQ. evidence of VD Review Right Lower quadrant Venereal disease

22 IPPB tx Q2h for CF. Review Intermitten positive pressure breathing Treatment Every 2 hours Cystic fibrosis

23 TPR and B/P tid Review Temperature, pulse, respiration Blood pressure Three times a day
