3 The WIIFM factor We are promoting a new relationship between the state and the citizen, but what is in it for me?

4 What is a support plan?  It is the plan of how the individual is going to spend their personal budget – therefore they will need to know how much their personal budget is to be able to make their plan.  It is a person centred document that will be shared with the council, therefore it will be a public document and needs to say as much as is needed but no more than necessary.

5 Who can help you to plan? Support planning can be done by any-one and everyone, it isn’t something a professional does for you, it is a person centered process which should be done WITH you.

6 Some ideas to develop You can do your plan yourself or you can get help to do your support plan from:-  Family and Friends  Peer support – other people who are in a similar situation to you but have already done their plan.  A local community organisation  A Provider organisation you are familiar with.  Your care manager, social worker or someone else from the council who offers to help you.  An independent broker

7 What sort of things should be in the support plan  Who are you and what is important to you.  What are your plan’s for the future, what do you want to change or achieve  How will you stay healthy, safe and well.  How are you going to organise and arrange the help and support you need.  How you will stay in control of your life  How will you manage your support  What will your support cost  What are you going to do to make this plan happen

8 How creative is creative?  Your support plan can be done in any medium – but should be done in a way which makes sense to you. It also needs to answer some questions very clearly so the council can decide if your plan is going to enable you to meet your outcomes.

9 The hardest things about your support plan, is making it happen  Then it is about how you keep it alive and working.  What do we need to learn through the review process.  What happens when things go wrong?

12 Review and Learn Find out what outcomes people have acheived. How can we make the customer journey more simple but equally as effective? Who are the best people to help people?

13 We know that investing in individuals, families and communities works and provides a sustainable solution. BUT first you must believe and secondly you must give it time to work. It ain’t no quick fix

14 Exercise. In Social work observation is important as it is in support planning. I am going to put a picture up for a minute then I want one person from each table to describe it.
