2 What is a Growth Plate? Areas of cartilage located near the ends of bones Found on long bones Between widened part of the shaft of the bone and the end of the bone Long bones grow out from growth plates When a child is fully grown, the growth plates harden into solid bones 15% to 30% of all childhood fractures are growth plate fractures

3 Classification of Growth Plate Fractures Salter Harris System Type 1 - Breaks through the bone at the growth plate Type 2 - Breaks through part of the bone at the growth plate and cracks through the bone shaft, This is the most common form Type 3 - Cross’ through a portion of the growth plate and breaks off a piece of bone end Type 4 - Breaks through the bone shaft, the growth plate, and the end of the bone Type 5 - A break due to the bone being crushed

4 Causes of Growth Plate Fractures Often caused by a single event, such as a fall or car accident. They can also occur gradually as a result of repetitive stress on the bone. All children who are still growing are at risk for growth plate injuries,but there are certain factors that may make them more likely to occur: -Growth plate fractures occur twice as often in boys as in girls, because finnish growing earlier than boys. -The incidence of growth plate fractures peaks in adolescence. - One-third of all growth plate fractures occur during participation in competitive sports such as football,basketball, or gymnastics.

5 Symptoms of Growth Plate Fractures A growth plate fracture usually causes persistent or severe pain. Other common symptoms include: Visible deformity, such as a crooked appearance of the limb. An inability to move or put pressure on the limb. Swelling, warmth, and tenderness in the area around the end of the bone, near the joint.

6 Treatment Treatment for growth plate fractures depends on several factors,including: 1)Which bone is injured 2)The type of fractures 3)How much the broken ends of the bone are out of alignment(displaced) 4)The age and health of child 5)Any associated injuries

7 There are two types of treatment: Nonsurgical treatment Growth plate fractures can heal successfully when treated with immobilization Doctors most often use cast immobilization when the broken fragment of bone are not significantly out of place A cast will protect the bones and hold them in proper position while they heal

8 Surgical treatment Most common operation used to treat fractures is called open reduction and internal fixation During the procedure, bone fragment are first repositioned into their normal alignment(called the reduction) Bones are fixed with special implants like screws and wire, or by attaching metal plates to the outer surface of the bone A cast is often applied to protect and immobilize the injured area while it heals

9 Recovery A child's bones heal quickly, but a growth plate fractures can still take several weeks to heal. After the bone heals, your doctor may recommend specific exercise to strengthen the muscles that support the injured area of the bone and improve the range of motion of the joint.
