Outreach and Recruitment Youth and young adults (age 13 – 34) living with HIV/AIDS who: are aware of their HIV infection status, but have never been engaged in care; are aware their HIV infection status, but have refused referral to care; have dropped out of care; are in care but have not achieved viral load suppression; inclusive of people with disabilities, minorities, limited health literacy and LGBT persons.

Outreach and Recruitment Roughly 10% of national HIV/AIDS population resides in NYS 77,000 of the 123,000 are virally suppressed Our app is targeting the 37% whom have not reached viral suppression or are not linked to care HIV/AIDS in New York State

Determining Eligibility Age 18 to 34 Living in New York State HIV positive Have a smart phone, tablet, ipod touch running IOS or Android. Are receiving care at either PFY or Northwell Speak English or Spanish Have Capacity

Screening and Enrollment Process Through outreach potential participants will be identified The PEEP will be notified and will connect with each person IdentifyScreen Enroll Potential participant will answer 9 eligibility questions If eligible, PEEP will guide through evaluation survey, “GET” app download, tutorial of app features, and send first inter-app message Link
