Steps to Business Registration and Licensing with OTR  Corporate Registration with DCRA (Department of Consumer and Regulatory Affairs) if incorporated  All businesses must register using the FR-500  The FR-500 can be completed online @ www.taxpayerservicecenter.com or postal mailed, emailed, faxed or hand delivered to our walk-in center. www.taxpayerservicecenter.com  The Notice of Business Registration is issued instantly (provided no errors on the FR-500)  FR-500B Special Event Registration register businesses for Sales and Use tax, and are due a reasonable time in advance of the scheduled event.

Common Errors  Omitting SSN for the officers  Omitting the business code (NAIC)  Using the SSN as the EIN  Business Name listed under the SSN instead of EIN  After registering, changing the business name without registering the new name with DCRA (only if incorporated). A new FR-500 is not needed to change the name  Registering without including an authorized person(s)
