A generic term to encompass all those occasions in which a planned sequences of mental or physical activities fails to achieve its intended outcome, and when these failures cannot be attributed to the intervention of some chance agency. - Intention -Error due to planning process or to execution

15 Final Exam—Example Answer #4 The data that managers have should be accurate and complete and meet their needs. Assessment of the data provided should include being able to track trends and analysis specific data to improve processes that are needed to improve quality management. In the medical field, specifically the nursing department of a given hospital or facility, this information is very important to decrease errors, medical and physical. It also should improve patient quality of care by analyzing what has happened in the past. This data is helpful in any other departments for an increase in the ability of their areas to be free of errors and increase the quality of their services.

16 Final Exam—Example Answer #4 In marketing, this data is useful to see the trends of the environment or to provide the organization the opportunity to see and analyze their processes and areas for improvement. The processing of the data that is provided from CQI/TQM processes is beneficial to all areas of an organization because each department in a facility is important to meet the needs of the market, provide quality care and services to the people they provide for. If in this economy, a facilty does not stay one step ahead and use the information given to them to the best of their abilities they will not survive in today’s market.
