4 4 Disability. What is it? What is a disability? Generally speaking, it means you are unable to work in your occupation as result of: Illness or Injury Could this happen to you? Could you be absolutely certain you will never experience an accident or suffer a sickness? If not, Could you experience a disability?

5 5  How would you pay the bills if you were disabled? A disability could last for weeks, months or even longer. Would you: Be able to rely on your savings? Borrow from friends or family? Run up credit card debt?  Would medical insurance, workers’ compensation or Social Security disability pay for your disability? The answer is NO in most cases. What would you do if your paycheck stopped tomorrow? Regardless if you live paycheck to paycheck, this insurance can be a very important financial safety net.

7 7 It’s important to know your policy’s definition of disability  You are unable to perform the material and substantial duties of your regular occupation;  You are not engaged in any occupation; and  You are under a physician’s care for the injury or covered sickness causing such a total disability Unum determines whether you have an eligible disability due to sickness or injury based on this definition

8 8 You can choose from the following options: Benefit period: [Indicate the benefit period offered according to specific plan design offered] Elimination period: [Indicate the EP options according to the specific plan design offered] Benefit amount: [Indicate the benefit amounts available based on the specific plan design offered – adjust if necessary] Choose a monthly benefit between X and X-X for an illness and/or off-the-job injury disability. Coverage of up to 60%* of your gross monthly salary may be offered. *Benefit is limited to 40% in CA, HI, NJ, and RI.

9 9 [Mental illness rider This rider provides a disability benefit due to mental illness equal to 50% of the policy benefit amount, up to a maximum benefit period of 24 months.] What else is included with this benefit? You may keep your coverage if you leave the company or retire. You can keep this policy up to age 72. If you become disabled and are receiving benefits, you won't have to pay for this coverage after 90 days of total disability. Unum will waive your cost until you recover and return to work.

10 10 Pre-existing condition limitation What is a pre-existing condition? It’s a sickness or physical condition for which within 12 months before the coverage effective date: 1.Symptoms existed that would cause a person to seek advice or treatment from a doctor; or 2.You were treated, received advice from a doctor or took prescribed medicine. How can it affect you in the event of a disability? 1.If you experience a disability because of a pre-existing condition, benefits will not be paid for such disability if it happens during the first 12 months your policy is in force 1 2.If your disability due to a pre-existing condition occurs beyond 12 months from the effective date of coverage, your disability will be treated as any other disability 1 –Six month period applies in ID and NV. In TX, six month period applies only for applicants 65+ Coverage effective date Pre-existing limitation no longer applies [12] months †† after effective date [12] months †† before effective date

11 11 Are disabilities due to maternity covered? Pregnancy 1 Nine months after coverage becomes effective, pregnancy is considered the same as any other covered illness. The monthly benefits due will be paid once the elimination period is fulfilled. What if birth occurs within the first nine months?  Benefits will not be paid if the insured gives birth within the first nine months after the effective date of coverage. However,  Medical complications of pregnancy may be considered as any other covered sickness, unless those complications were pre-existing. 1 Nine-month giving-birth exclusion is not applicable in CA, KS, MT and OK.

12 12 Why purchase short term disability coverage? 1.It pays a portion of your gross monthly salary if you cannot work due to a covered injury or illness. 2.Affordable premiums are based on your age on the policy effective date. 3.You can pay for it through convenient payroll deduction. 4.[Guaranteed issue coverage for 60% of income up to $3,000 with the greater of 20% participation in the plan or 10 applications.[ 5.[Modified issue underwriting for coverage amounts of 60% of income up to $3,000 of covered salary (requires three questions to be answered)]. 6.[Simplified issue underwriting for coverage amount of up to 60% of income up to $5,000 of covered salary (requires additional questions to be answered)]. 7.Coverage becomes effective on the first day of the month in which payroll deductions begin.
