8 011005 version8 ERSC Eastern Region Service Center The ERSC provides case management services to assist the individual to establish and maintain their course of treatment, achieve long-term recovery and make a smooth transition back to the community. Participation in the program is voluntary and usually lasts between three to six months. Services include: Assessment Advocacy Securing Entitlements Engagement Basic Needs Assistance Treatment Linkages Monitoring Outreach Referrals for housing and vocational assistance

9 011005 version9 ERSC Eastern Region Service Center Target population for case management services includes: Adults with behavioral health needs (i.e. Diagnosis of a mental health and/or substance abuse disorder; substance abuse must be primary) Living or originating in the Eastern Region On State Administered General Assistance (SAGA), SAGA eligible or indigent Have specific case management needs, often related to clinical services, entitlements, return to work or housing

11 011005 version11 CPAS Community Prevention & Addiction Services Community Prevention and Addiction Services, Inc. (CPAS) provides a continuum of substance abuse treatment: Transitions Outpatient Services located in Willimantic, Danielson and Manchester provides evaluations, individual and group treatment. 860-456-3215 Pathways, located in Putnam, provides medically monitored detoxification in order to prevent the serious side effects of withdrawal. A methadone protocol is available for heroin detoxification. 860-963-4971 Milestone Intensive Residential Program located in Putnam highly structured and rigorous residential intensive treatment for chemically dependent persons who are in need of a highly structured environment. The usual clients who attend the Milestone Intensive Program are in the early stages of recovery. The 18 bed Milestone provides co-ed treatment Intensive co-ed facility is an intensive counseling milieu with a variable length of stay based on treatment needs. 860-928-1860

12 011005 version12 CPAS Community Prevention & Addiction Services Thomas Murphy Center located in Willimantic, provides residential intermediate treatment for people who are in need of a structured environment to continue their recovery and who have developed the skills necessary. The program offers the opportunity for clients to seek employment actively in order to begin the process of reintegrating into the community. after residential treatment. Clients of this program focus on employment and/or educational needs during the day and return to the facility in the evening for substance abuse treatment. 860-456-1769 New Life Center, located in Putnam, provides residential substance abuse treatment for chemically dependent pregnant and postpartum women and their children. Women who enter this program need the structure of an intermediate program and support to help them with additional stressors of pregnancy and/or raising newborns/toddlers. 860-963-6763
