4 Baseline assessment 1.3 Carer views and knowledge sought throughout the assessment and treatment process. 1.6 Treatments and strategies for medication management are explained to the carer 1.7 Carer has access to advice re advocacy, equipment and welfare rights. First steps: Develop an in-patient forensic Carer Champion network for the service. Define Carer Champion roles

7 Leaflet On admission Full assessment by the Multi Disciplinary Team (MDT) leading to an individual treatment package. Responsible Clinician: is responsible for the care and treatment of your family/friend Allocated a named nurse: who will coordinate your family or friends care. Carer champion: who will be available to speak to you. The weekly care package and progress will be reviewed weekly by the MDT Care Programme Approach (CPA) meeting within 3 months to review the whole care package and plan for the next period of care CPA every 6 months Prior to discharge – planning meeting
