2 Who are we? Statutory organisation Responsible for 188, 000 registered patients and an estimated day time visiting population of up to 1m people Work together with West London, Hammersmith & Fulham and Hounslow CCGs – the CWHH Collaborative Small team dedicated to Central London CCG, with a larger team shared across the CWHH Collaborative, supported by the North West London Commissioning Support Unit 2

3 Winter Pressure Plans Role of providers Our Commissioning Support Unit (monitoring) Additional funding to ease winter pressures: – 7 day access to primary care – Invest in step up/down beds, extending therapies and re-enablement services to 7 day working, increasing capacity in rapid response team – Increase funding to acute trusts to increase clinical input to Urgent Care Centres, with 24/7 opening at St Mary’s and Hammersmith Hospital, increasing consultant cover in A&Es, increase patients seen via ambulatory care pathways and facilitate timely discharge, funding 7 day social care team in acute trusts – Funding to support 24/7 psychiatric liaison at Hammersmith Hospital Medical support to rapid response nursing team to help enhance the team’s ability to avoid hospital admissions (West London CCG) 3

4 Communications 7 day access to GPs (tri-borough) – Central London CCG to date - postcard, posters, phone boxes, schools, GP practices, libraries, Church Street notice boards, mail drop in Church Street Developing a winter pressures marketing plan across local CCGs – initiatives and keep people well over winter How you can help – Advice and ideas on how to best to reach the groups you work with – email helena.stokes@nhs.net 4

5 Improving Urgent Care Homeless outreach – working with Hopkinson hostel Groundswell – targeting high A&E attenders and supporting homeless people to stay well with primary care BME Insight Project – understanding how we can improve urgent care for BME communities Further insight work to understand why people choose services 5
