1 Virtua’s goals for PHR technologies Create tools and processes that: Encourage patients to be engaged with their healthcare Provide tools for caregivers of dependents Provide health information in a meaningful way Allow selected patient self-created data to be pushed/pulled with the Virtua record Support continuity of care, especially between inpatient, ambulatory and home settings Are secure Connect to home-health devices Support the Outstanding Patient Experience Build patient loyalty Meet new “Meaningful Use” regulations

2 Portfolio of PHR technologies Microsoft Amalga for data aggregation MS HealthVault – data storage Mayo Health Manager for primary user interface (re-branded as the Virtua Health Manager) Wellogic HIE platform for data exchange CitiusTech – Secure exchange of patient forms Zweena for capturing private paper medical records Getwell@Home – Extension of hospital stay care plans & patient pathways into the patient’s home

3 Solutions using PHR technologies E-data within HV/Virtua Health Manager: Procedures and encounters history Allergies Lab Results Discharge Summaries Medications, including immunizations Discharge Education Other benefits of the Mayo Health Manager Structured data from electronic Forms (Citius Tech solution) Scanned images of private paper records & related metadata (Zweena solution) At-home video educational content and plans of care (GetWell@Home solution)

4 Key benefits of the Microsoft PHR technologies Amalga – HealthVault relationship – easier means of integration HealthVault Linkages to many other 3-party data sources Easy of user authentication (Microsoft LiveID) Patient owns the data once it’s contained in HealthVault Cost-effective Virtua Health Manager – Provides a clinician-developed PHR tool, with respected guidance from experts at the Mayo Clinic and driven by USPSTF (US Preventive Services Task Force) and ICSI (Institute for Clinical Systems Improvement) standards

13 Virtua qualifies patients by querying Amalga (e.g., future surgery patients) and generates an e-mail mailing list E-mail is sent to patients with a hyperlink The hyperlink brings the patient to a specific form in a SharePoint document/forms library after LiveID authentication. Forms are web- based. Patient completes the form and hits Enter. Forms data (structured) is saved in Amalga for use by clinicians (a new “view”) and in the patient’s HealthVault account Patient data always remains secure through the transaction Secure Patient Forms How it works:

15 While structured data in PHRs is a long-term industry goal, it will take 5 – 10 years before most private physicians are using structured-data EMRs, and even then there will be residual paper. Zweena provides services and technology to store and present scanned paper medical records, via the Web. Zweena also abstracts data from the paper record to create metadata for patient use, and it has various consumer tools (e.g., health risk assessments) Patients use Zweena via a user account, maintained by Zweena Zweena’s database is synchronized with HealthVault LiveID is Zweena’s user authentication method Scanned Paper Records What and Why?

16 Scanned Paper Records How it works... Zweena will be an option presented on Virtua’s PHR “jump-off” website page Like other third-party applications, the patient will need to link Zweena with HealthVault and establish a Zweena account Zweena works directly with the patient for the patient’s authorization to obtain copies of medical records from physician offices (usually via facsimile) Zweena scans the records into Zweena’s data base Records are stored in Zweena’s data base and can be synchronized with HealthVault. Experiment: Virtua will subsidize patient’s cost for one year.

17 GetWell@Home (coming in 2011…..) The GetWell Network is Virtua’s in-house educational and entertainment system Patients can sign-up for GetWell@Home while in the hospital

18 GetWell@Home: Extends Existing Care Plans into The Patient’s Home Discharge Education Access to Discharge Instructions Health Status Self Assessments Request follow-up appointments Medication Teaching/Reminders Other Post Discharge Activities Virtua Patient Illustrative Examples

19 In partnership with Cablevision, GetWellNetwork is building the first interactive health channel for 3 million homes in NY-area GetWell@Hospital GetWell@Home Cable TV Network: Secure network and services to connect the health care ecosystem
