2 Acknowledgement Form The first step in a successful crisis management plan is ensuring that all employees read the plan and know what to do should a crisis arise. Employees should sign the acknowledgement form to show they have read the entire crisis management plan and are aware of their responsibilities in a crisis.

4 First Action Enact this plan if our relationships with current or future guests, our loyalty program members, our employees, our shareholders, Wyndham Worldwide, or the City of Dallas are at risk. Do so by immediately contacting the Crisis Management Team leader on the emergency personal line at (214) xxx-xxxx or by coming to his/her office. Should the CMT leader not be available, please contact the hotel’s Marketing and Public Relations Director at (214) xxx-xxxx or his/her office.

8 Proprietary Information It is imperative that the information contained in this report be kept confidential. Do not release any of this information without prior written consent from senior executives or legal counsel.

9 Crisis Management Team Communication Strategy It is also important to keep track of all communication sent out during the crisis. Please use this sheet to record who was contacted and the purpose of that contact. Remember to attach a copy of the actual communication as well.
