Your care will not be affected by race, religion, beliefs, cultural values, sex, age, or financial status. ask all personnel involved in your care to introduce themselves, state their role in your care, and explain what they are going to do for you. Information about Treatment You can expect your healthcare team to explain: a concise description of your condition and proposed treatments, as well as your role in your care and the knowledge and skills you need. alternatives of treatment. the prognosis and any problems relating to recuperation. the benefits and risks of treatment. Participation in Decisions about Your Care We respect your right to: informed consent. Voice Complaints You have the right to: voice complaints regarding the quality of care and services your receive, and be assured that your presentation of a complaint will not compromise your access to care. You, in Turn, Have the Responsibility To: Provide Complete Medical Information Provide, to the best of your knowledge, accurate and complete information about your present health status, your complete medical history, including illnesses, hospitalizations, medications, advance directives, and other matters related to your health. Make Informed Decisions and Ask for Clear Explanations We encourage you to gather as much information as you need to make your decisions about your health- care. We encourage you to ask for clarification if you do not understand your diagnosis, treatment or your role in your healthcare. You may be asked to consent in writing to certain tests or procedures. Ask as many questions as you need to fully understand each document you sign. Report Changes Tell your physician about changes in your health. If you are seeing another physician and they have changed your medications, please inform your physician. We have to approach your care as a team. Accept Financial Obligations Ensure that your financial obligations to our practice are fulfilled as promptly as possible. Respect the Privacy of Others It is important to be considerate of other patients by observing their right to privacy and maintaining a quiet atmosphere.
