5  Background: DESMOND evaluation  12 month pilot project April 2014  Type 2 diabetes education and Ramadan campaign  BME Community  Group work on Conversation Maps  Bilingual group workers and health trainer, ideally bilingual health lead  Sessions in community/segregated  12 sessions each 3h each, 2 maps

6  Type 2 diabetics in last 5 years  BME Groups  If offered and declined/unsuitable for DESMOND  Referral via Practices & Self Referral  Clients and their carers  30 people per session  Evaluation: Over to Peta  Next steps: Identifying clients and packs

9  Before & after course questionnaires: ◦ Revised Michigan Diabetes Knowledge Scale; ◦ DESMOND end of course evaluation?; ◦ Or Conversation Maps evaluation?;  These assess the attendees satisfaction with the course and it’s quality;  Electronic anonymised clinical data will look for improvement in clinical outcome measures;  Value for money assessment – ongoing funding;  Reports after: 2 sessions; 6 months & 12 months, to monitor progress and adjust programme based on course facilitators and attendees input.
