23 exploreengage elevate Leader of the Year Award: Chapter leaders will need to be nominated by chapter attendees (users AND/OR partners) by filling out the following form: https://www.surveymonkey.com/s/leaderoftheyear2014https://www.surveymonkey.com/s/leaderoftheyear2014 This survey contains 3 questions that need to be completed in order for a chapter leader to be eligible for the award: How does the leader go above and beyond in their role? Describe what makes this leader effective in their role. Please describe how the leader effectively carries out the mission of CRMUG. An internal committee at CRMUG will review the collected forms, and take into account other information including: leader's attendance on leadership calls, actively posting to Collaborate, number of times chapter appears on Top 10 Quarterly results, holds a minimum of 3 meetings in 2014 (4 th meeting can be scheduled into Q4), participates in additional CRMUG volunteer activities, ie: speaking at Summit, webinars, etc. Multiple nominations WILL be accepted and taken into consideration during the selection process by the committee. Nomination Submission Deadline: Friday, September 12, 2014
