Rationale A number of large trials have confirmed the benefits of thrombolysis in acute stroke, but there are gender differences in stroke. Methods & Results Ebrictus Project is a study based on a cohort population from incident cases on the first stroke episode (included TIA) from 01/04/2006 to 31/12/2014 participated by Primary care and neurological service referent.. Variables collected: Framingham, Regicor, CHA2DS2-VASc, Essen, NIHSS, Barthel scales and survival outcomes according to gender by Kaplan-Meier’s curves, and Cox multivariate Statistical approaches for analyzing survival outcomes and their relation with thrombolysis therapy. Results A total of 1337 first-ever strokes were included (614 women). Mean age 74.06±11.9 years (25-90), higher percentage (p 0.004) \s women than men (54.1%), and significant differences (p <0.001) in the mean age men (72.34±11.9) and females (76.09±11.7) and significantly older (p <0.001) than those in the thrombolysis (68.8±12.0) The group of men has a HIGHER baseline cardiovascular risk and a percentage of active smokers significantly higher than women (p <0.001). The arterial hypertension (RR 12) and atrial fibrillation (RR 8) have the higher relative risk for experiencing a first episode of stroke. The age-adjusted death rate (37.1/104 CI95% 26.2-49.3) among men is significantly higher than in women (14.7/104 CI95% 7.7-21.8). Around 40% of strokes were diagnosed simultaneously with AF undiagnosed previously. The average age at the diagnosis of AF was 73.65±8.0 years. If we consider the expected AF prevalence, it stands out the AF cases aged 70-74 years had the bigger proportion (51.2% CI95% 47.8-54.5) with unknown AF and no treatment OAC. The 8.6% of ischemic stroke were treated with thrombolysis. Their mean age was 69.3±10.3 years. The realization of thrombolysis (CI95% 0.37-0.80 p 0.002) and Barthel score ≥60 (CI95% 0.81- 0.94 p 0.002) score were identified as protective factors of mortality. The incidence density of mortality for the group was 8.5/100 cases/ year follow up (CI95% 5.4-12.8); in the group of women was 4.2/100 cases/year (CI95% 1.5-9.2); and men, 13.4/100 cases/year (CI95% 7.8-21.5) and was statistically significant (p 0.01) and showed an increased risk in men compared to women IR = 3.2 (CI95% 1.2-8.0). The probability of survival among women was 0.81±0.06 and among men 0.43±0.02 (p 0.012). The adjusted YPLL <70 years was significantly higher in men (12.0/104 CI95% 10.7-13.3) than women (6.5 CI95% 5.6-7.4 6.5). Conclusions On one hand, the men between 65-74 there have higher cardiovascular risk and higher mortality rate than women, and the cardiovascular prevention doesn't seem have the best one. On the other hand the exposure to thrombolysis in women does seem much more protective than men. Key words: Stroke. Thrombolysis. Community based study. Mortality. Survival.
