 “Cohort review is a systematic review of the management of patients with TB disease and their contacts.”—Understanding the Cohort Review Process: Instructional Guide (2006)  “The fundamental concept of cohort review is accountability. Staff are accountable to supervisor and to the program for how well they are caring for patients…and the program is accountable to patients and to the public for controlling tuberculosis.”—Dr. Thomas Frieden, MD, MPH, New York City Commissioner of Health

 Requirement under the TB Cooperative Agreement  Adaptable

 NTIP objectives included in county contracts—NY state has contracts with counties that average >5 cases/year  Performs cohort review at contract site visit (kill 2 birds with one stone)  3 largest counties do two reviews/year: one at the site visit and one on their own  Objectives included are: COT, Sputum Culture Conversion, HIV Status, and CI  Cohort review for counties w/o contracts is done 2x/year by regional staff via conference call with state Central Office Staff (same objectives reviewed)

 Contract Addendum with all 100 counties  Goals are at or above the NTIP objective goals  Cohort review is performed for all high morbidity counties  Objectives included are: COT, Sputum Culture Conversion, HIV Status, and CI

 Contracts with all metropolitan regions (6)  Quarterly cohort reviews are down with Memphis/Shelby County (video conference) and Nashville/Davidson County (in-person)  NTIP objectives are calculated for each individual patient and for the entire cohort  Objectives included are: COT, CI, Treatment Initiation, Sputum Culture Conversion, Recommended Initial Therapy, Genotyping, HIV Status, Sputum Culture Results Reported
