MiPCT ADTs: From Alert to Care Delivery Care Mgr (or PO) checks system a minimum of once per weekday Care Mgr Receives Electronic Alert Care Manager accesses available information (e.g., health system EMR, discharge summary, etc.) Care Manager communicates with facility discharge planning contacts or via call to patient Care Manager accesses available patient info. Medication reconciliation and TOC activites Identification of patient needs and initiation of follow-up care Bill (G/CPT) if applicable Initiate Near Real Time Transition of Care Activity 2

. What is the Wave Two Opportunity? The MiPCT has contracted with PatientPing (at project cost) to deliver ADTs to interested POs for MiPCT patients. Option (option not a requirement) for MiPCT POs and practices Provided at no cost to the PO or practice for MiPCT patients – the fee is paid by MiPCT administrative funds PO ADT Partners who participated in the Crimson ADT partnership were given the first opportunity to join PatientPing (a.k.a., “Wave One” Participants). Funding is still available for interested POs. First come (with signed agreements), first served.

. What Does PatientPing Offer? ADTs for all hospitals feeding ADTs to MiHIN Availability of post acute care transitions (SNFs, home health agencies) Opportunity for POs to define messaging (Point of Care Guidelines) that help the facility know who to contact re: the patient’s transition Training and support (through PatientPing/MiPCT partnership) on how to use the system and, if desired, on workflow

The PO ADT Lead Role Each PO who signs a participation agreement is asked to appoint two PO ADT Leads who: –Participate in monthly ADT User Group Calls –Facilitate training of CM and other team members
