IN CONCLUSION…. Most of the physicians at Select Specialty Hospital – Cincinnati sign, date, and time their orders. Out of the few who didn’t do all three, they signed and dated. One solution would be to remind the physicians that signing, dating, and putting a time on their orders is important. It is important because detailed documentation is vital for quality and safety.

IN CONCLUSION… (CONT.) “While the ultimate goal is delivering safe and improved patient care, it cannot be ignored that errors made during hospitalization result in greater costs (such as increased length of stay, need for higher level of care, and additional procedures).” (AHIMA. "HIM Functions in Healthcare Quality and Patient Safety.“) So not only is proper documentation important for quality and safety. Proper documentation can also save money.
