“Transition services are a coordinated set of activities for a student with a disability that is designed to be within a results oriented process, that is focused on improving the academic and functional achievement of the student with a disability to facilitate the student's movement from school to post school activities”

Transition Services Include … Postsecondary education, vocational education, integrated employment (including supported employment), Continuing and adult education Adult services, independent living, or community participation “Based on the individual student's needs taking into account the student's strengths, preferences, and interests.”

Typical transition goals Student will complete career inventories. Person responsible: Parent, in conjunction with school district personnel. Student will visit Community College and the Office of Disabilities. Person responsible: Student and parent.

Typical transition goals Student will be referred to for information regarding post-secondary education and training options. Person responsible: Parent. Student will investigate possible job shadowing opportunities Person responsible: Student and staff.

Typical transition goals Student will participate in the ASVAB assessment to determine aptitude.
