CONCEPT create a semi-hospital facility:  close or within the hospital  in which discharged patients feel like home  in which nurses and doctors can easily intervene in case of emergency  with daily management costs lower than hospital stay

MOTIVATION ~8% of hospitalized patients are potentially dischargeable but they end up staying in the hospital for different reasons [ Lenzi et al BMC Health Services Research ] Frailty index of elderly increases after 12 days of hospitalization Inefficient occupation of hospital beds, increases hospital costs (~1000€/day)

EXPERTISE REQUIRED Clinician  select patients category  define inclusion criteria in the study  choose parameters to be monitored Architect  design comfortable environment  conceal all ICT devices required to empower the patient Engineer  Collect measurements of clinical parameters through non invasive methods  develop integrated smart systems able to assess patient health and help clinicians in the early prediction of health risks
