Reaching across Arizona to provide comprehensive, quality health care for those in need

 Short Term Goals Reduction of ED visits by 10% Development of a care management protocol  Long Term Goals Improve provider and member engagement Development of preventive model

 Weekly RBHA/Contractor interdisciplinary meetings  Monthly doctor to doctor intervention with team meetings  Share of real time data  Mutual Care Plan  Providing support services such as vocational rehabilitation, housing specialists, and peer supports  Pediatric Check-Up protocol to coordinate care

 Awaiting data feed from AHCCCS  Contractors have varying analytical capabilities  Related projects requiring data exchanges Blind Spot Data provided by AHCCCS Super Utilizer Data Exchange/Care Coordination between Contractors and RBHAs Psychotropic Oversight and Monitoring  Lack of a single, standardized process and joint goals for the RBHAs and health plans Identified Potential Barriers

 Director Betlach requests that AIHP become an active participant in the Health Plan/RBHA collaborations  Elizabeth Carpio is the new AIHP Care Coordination Leader (  All the RBHAs were advised to provide Ms. Carpio with a RBHA contact person by March14th

 Ongoing meetings between the RBHAs and health plans are scheduled  Meeting summaries will be presented to AHCCCS and DBHS Executive Management in July, 2014  It is the expectation that these summaries will include AIHP in the collaborations
