2 Articles Problem Fire swept the Command Module during a preflight test - killed 3 astronauts. Fire began because of electrical short, spread due to volition atmosphere and amount of combustible materials. Hatch design and management also contributing factors Apollo 1 NASA and the US government wanted to put men on the moon by the end of the 1970's Ethical Dilemmas Steakholders pressured team to minimize cost Combustible materials used throughout shuttle Also pressured to reduce time Rushed through preflight check ignoring smells and alarms Results Changed how NASA communicated and managed missions Restrict combustible materials Safety = #1 concern Emergency personnel available
