What is a Coordinated Set of Activities? Activities, based on student needs, that will move the student toward accomplishment of his/her post secondary goals Persons responsible for performing the activity are identified, as well as when it will be completed

Think of It As An ACTION PLAN! WHAT? WHO?BY WHEN? What activities Who is When will need to be responsible? they be accomplished? done?

A Coordinated Set of Activities Includes: Instruction Related Service Community Experiences Employment & other post-school objectives And when appropriate: Daily Living Skills Functional Vocational Evaluation

Let’s Take a Look at an Example Henry Forbes, Age 15 – 10 th grade Post-Secondary Goal for Employment: After graduation, Henry will obtain employment in the graphic arts field. Post Secondary Goal for Education: After graduation, Henry will attend a 2 year college and major in graphic arts.

All About Henry: Strengths: o good hygiene & grooming o good listening & verbal skills o creative and artistic o good people person Preferences & Interests: o likes art o likes working on the computer Info. from ESTR-J, teacher interview and Post-Secondary checklist: o feels good about self o completed intake for DARS o schedules own appointments o treats minor medical problems o corresponds in written form o does own calculations with calculator

Henry’s Action Plan WHAT?WHO?BY WHEN? 1.Visit college/career counselor on campus and ask questions about graphic arts field and possible college choices. 2.Take Vocational Experience class and Building Workplace Success class from VAC teacher. 3.Begin leading ARD own ARD meeting, after preparing and practicing with case manager. 4.Take Math of Money class as elective. 5.Open up bank account and practice writing checks and using ATM card with allowance tied to completing chores at home. 6.Take Driver’s Training course and take test for Driver’s License Henry Mrs. Forbes Henry Case Manager Henry Mr. and Mrs. Forbes Henry Mr. and Mrs. Forbes By end of Sophomore year In Junior year Start at upcoming ARD in January 2011 During senior year Henry Mr. and Mrs. Forbes Summer of 2011
