 What happened: Initial launch of vessel Immediately rolled to port and capsized 6 persons went into water and were recovered Vessel sank in marina  Salvage Master/SERT believe vessel has design flaw and is unstable as designed.  Salvors attempted to refloat – vsl rolled to port  NTSB also involved in investigation  Lifted with large crane barge and placed on land

WHAT HAPPENED:SIGNIFICANCE:  Unknown cause of fire  8 vessels caught fire  6 vessels sank  Used OSLTF to conduct initial response/mitigation of fuel spill  Insurance ultimately took over contract for cleanup  Estimated 1500 gals mixed petroleum products discharged  1350 gals recovered  Outstanding multi-agency/tribal response and coordination  Marina staff very proactive / supportive  Prolonged response with good outcome

WHAT HAPPENED:SIGNIFICANCE:  Vessel grounded attempting to enter Snohomish River  Vessel was being delivered to buyer as part of the sales contract  Wooden vessel built in 1939  Operator unable to pay for salvage  Opened OSLTF and coordinated w/ACOE and WA DNR  COTP order issued for lighting and tow plan  Log tow vessel complained about impeding commerce  On water recovery/containment challenging due to location  ACOE would not remove  WA DNR ultimately able to pay for removal  Challenging issues w/determining RP

 What happened: Pump malfunction on waste oil tank receiving ballast water from sub Initial report was 50 gal Later determined to be up to 2000 gals of oily water mix High media/political interest initially Significant sheening in Hood Canal Opened OSLTF for CG responders Navy stood up full ICS Unified Command Secured “Delta Pier” provided significant challenges for responders Brought in Navy spill response contractors Brought in “seasoned” commercial spill responders to coach
