The management and prevention of disease processes will better help the chronic issue of diagnosis Disease management is more focused on acute problems, leaving an open space for error and time delay on chronic illness People with chronic issues are served better with an approach that is structured and organized towards ongoing assessment of the disease

 Disease management programs were developed because of the rising cost for HMO’s  Only a quarter of Americans are affected by chronic illness, yet accounted for 75% of spending  Disease management programs were put into place mainly because of the rise is COST

 Chronic disease is projected to increase by hundreds of millions every 5 years  Technology is so advanced today but the management of chronic disease is still an issue on the rise year after year  Systematic approaches are put into place but the problem still exists, why?

 Currently heart failure, diabetes, and asthma are the most common chronic illness’s but yet are most manageable?  Multidisciplinary approaches are needed in order to manage these chronic disease’s  The future of DMPs needs to remain systematic but improve in time management and the traffic of info to better improve the illness

 Starting at birth you are put into a system, in order to be tracked and logged on health  Continue with Obama care having “Healthcare” being accessible and affordable to everyone despite poverty or wealth  Health education continue to be a focus, but starting younger not only when the individual is sick  Elaborate on “time management” and communication between: patient and healthcare team
