HELP Survey II Opiates and Heroin  The survey tool was developed by Credo Community Center for the Treatment of Addictions, Inc., located in Watertown, New York.  The unscientific survey is confidential and voluntary.  The survey was completed by new or already established clients at Credo, at Samaritan Medical Center Outpatient Addiction Treatment Center, and clients presenting to Pivot case managers located at Jefferson County Department of Social Services.  The majority of completed surveys were received from Credo.
