COURTNEY ARNOLD – DIRECTOR OF HUMAN RESOURCES 401k Full-Time employees may elect to contribute a portion of their pay on a pre- tax basis OrTran has the option to make a discretionary contribution on behalf of employees of 20% of the first 4% each employee contributes.

ELIGIBILITY You are eligible to contribute to the company 401k plan after employed for 6 consecutive months, then able to start participating at the next entry period of January 1 or July 1. If you were hired: Before July 1, 2012: You are already eligible to participate Between July 1, 2012 – December 31, 2012: You will be eligible to participate beginning July 1, On January 1, 2013 or after: You will be eligible to participate beginning January 1, 2014.

ROAD TEAM CAPTAIN GUIDELINES Duties of the Road Team Captains: Assist Management When management has a new process or tool to present to drivers, the captains may be requested to provide their thoughts and/or advice on that process or tool. Captains will offer new ideas to management as they are presented to them. Be available for special projects, such as helping with the production of new training videos, etc. Be available when needed to discuss special concerns and issues, such as accident prevention and accident review board.

Captains will welcome new drivers - Call the new driver’s introducing him/herself and welcoming the new driver to the family. Keep a watchful eye Be alert at all times for unsafe actions taken by other drivers and advise the driver and management of the situation. Captains will contact management with all concerns.

Captains will not Give legal, financial or personal advise as a company representative. Give the new driver the impression that the captain is management. Captains WILL refer drivers to HR if any questions or concerns are brought to them about: Benefits Harassment issues Threats against the company, other drivers, customers, etc.

DUTIES OF OTHER DRIVERS (NON-CAPTAINS): Other company drivers and contractors WILL: Contact the Captains with policy/procedure questions or concerns during NORMAL business hours. Contact Driver Managers or the Safety Director for normal business activities or emergencies. Other company drivers and contractors WILL NOT: Contact the captain during off hours. Contact the captain with personal problems. Expect the captain to act as management or make managerial decisions.

HOURS OF SERVICE New regulations as of July 1, 2013 for Property-Carrying Vehicles

LIMITATIONS ON MINIMUM “34-HOUR RESTARTS” Must include two periods between 1:00 am and 5:00 am home terminal time May only be used once per week

34-HOUR RESTART Next restart break must not begin until 168 hours (7 days) after the start of the last restart break Must include two consecutive nighttime rest periods from 1:00 am to 5:00 am

RESTART PROVISION

After June 30, 2013, a driver may not take an off-duty period to restart the calculation of 70 hours in 8 consecutive days until 168 or more consecutive hours have passed since the beginning of the last such off-duty period. The 168-hour period is counted from the beginning of the last prior restart.

30-MINUTE BREAK No CMV driving if more than 8 consecutive hours have passed since the end of the last 30-minute break Break may be spent off duty and/or in a sleeper berth Counts against the 14-hour limit May do non-driving work after 8 hours without the break

REST BREAKS May drive only if 8 hours or less have passed since the end of driver’s last off- duty period of at least 30 minutes. HM mandatory “in attendance” time may be included in break if no other duties performed

BACKING ACCIDENTS ARE 100% AVOIDABLE. THEY ARE 100% PREVENTABLE. Make sure your trailer doors are properly secured. Watch for people, vehicles, forklifts, overhead wires, tree limbs, & other objects. Use your mirrors and back up slowly using the driver’s side whenever possible. Watch your clearance on both sides through your side mirrors. If it is dark, get out and check the unlighted areas with a flashlight.

Falling below the terminal average for two (2) consecutive quarters will result in the following: Having your speed parameters reset to 62 mph (both cruise and foot) The speed parameter will remain at 62 mph until the driver achieves a score of 90 or above Falling below the terminal average for three (3) consecutive quarters will result in the following: Complete additional training on driving technique and fuel economy Additional adjustments to the truck parameters may also be done at this time to assist the driver in meeting a minimum score of 90 or above.
