Program History 1998 – MiCTA provides E-Rate training for its State of Michigan K-12 and Library members 2000 – MiCTA and Michigan partner MiCTA files for E-Rate on behalf of the State of Michigan using MiCTA’s buying power and E- Rate to provide a streamlined purchasing approach for the State’s K-12’s/Libraries _7.pdf 3

In 2003 MiCTA files and receives its first Certified 470 (P2) on behalf of Members for Video Systems, Products, Basic Maintenance In 2005 MiCTA files its Second Certified 470 for its Members for Telecom/Internet Services In 2009/2010 MiCTA files 3 National 470’s on behalf of its Members for 1 – P1 and 2-P2 Services and Products 4

Mini - Bid Why is it needed? – Because MiCTA is a National Organization it provides its E-Rate members the option of multiple MSA’s (Master Service Agreements) What is required – If the service you are seeking is provided by multiple MiCTA Vendors members you should make sure you have evaluated your choice of a vendor based on the best service/product pricing/value offering for you – USAC requests that this process is documented 7

MiCTA Provides Information on MSA’s To USAC During your 471 Filing PIA Review MiCTA will provide any MiCTA 470 process/procedure document required by your PIA At your request MiCTA will engage directly with your PIA to resolve issues MiCTA retains all 470 process documentation (available to you if you are audited) NOTE: Starting with the filing window, as part of their review process, PIA’s will begin asking Members to provide proof of MiCTA Membership during the review process. This is a collaborative effort between USAC and MiCTA to assure member 471 filings are processed in line with the normal review process. Please retain the electronic response you receive from MiCTA when you join. If you are unable to find the information please contact the MiCTA office and they will assist in providing the information directly to your PIA reviewer. Any School or Library who chooses to use a MiCTA 470/MSA must be a MiCTA Member prior to filing their 471(s) or their application will be denied. 8

MSA’s Cover all Eligible Products/Services The combined request for Services/Products relative to MiCTA’s 3 Active Certified National 470’s cover all Eligible Products and Services contained in the USAC Eligible Services List 9

All 470’s Request Discounts MiCTA has requested pricing discounts for all 3 Certified 470’s That means that You have the option of either discount process: – SPI (Service Provider Invoice) Form 474 Sent to USAC by the SP to Obtain the Discount – BEAR (Billed Entity Applicant Reimbursement) Form 472 Sent to USAC by the Applicant to Obtain the Discount 10

Changes to MSA’s Per language contained in all current MiCTA Certified National 470’s: “Due to Evolving Technology and Potential Price Reductions, MiCTA Reserves the Right to Amend the resulting Contracts Over Their Term” MiCTA’s MSA’s (Master Service Agreements) are all “Living Documents” providing you our E-Rate Members with access to the most current technology and aggressive pricing. 11

RFP Process/Vendor Options With the support of the MiCTA Board and its Membership, MiCTA files its 470’s in concert with its own internal RFP process assuring the service/product/pricing value made available to You our E-Rate Members is based on the full buying power of the entire organization. As a national organization, we mirror our internal and E-Rate 470 MSA’s (Master Service Agreement) negotiations to provide multiple vendor options, when possible, assuring our E-Rate Members across the country receive the best possible pricing for your respective service/product choices. 12

E-Rate Consultants In an effort to assist our K/12 and Library Members who are looking for the best possible E-Rate Consultant choice, MiCTA, through a public, competitive, RFP process has negotiated agreements with some of the most knowledgeable consultants in the country. These agreements contain Consultant Fee discounts for our MiCTA E-Rate Members. 13

Form 470 Application Number – Priority Covers all eligible Telecomm/Internet Services or – Priority Covers all eligible Products (except Video Offering Below or – Priority Covers Video Systems, Components, Basic Maintenance only Note: if you have your own 470 number and wish to use it please insert that number in #12 above and related Allowable Contract information to assist your PIA in the evaluation of your 471 request(s) (Follow all other information given below) 15b Contract Number – Number that appears on the Service Agreement provided by your MiCTA vendor which references the specific MiCTA MSA – 14

471 Block 5 Cont. 15c Funding Request Under a Third Party MSA – Check the box 17 Allowable Vendor Selection/Contract Date – If you use P1 Number /1/2010 Covers All Telecomm/Internet Services – If you use P2 Number /11/2010 Covers all Eligible Products (except Video Offering Below) – If you use P2 Number /1/2009 Covers Video Systems, Components, Basic Maintenance 18 Contract Award Date – Date You sign the MiCTA Vendor Agreement 15
