CLINICAL ASSESSMENTS The Process

Purpose of the Clinical Assessment To present a clear and realistic picture of the client and their environment To provide a context for the client’s current functioning To highlight client strengths To present treatment recommendations/directions ( addressing where client is struggling)

THE FORMULATION An opportunity to “massage” empathy and understanding of the client through recognition of and focus on the underlying issues behind client functioning/behavior

SERVICE PLAN To highlight current goals To advocate for particular services

PROGNOSIS To focus treatment in a positive direction To add hopefulness and optimism to treament within a realistic context

CONTRACTS To clarify treatment directions for all parties To base treatment plan in reality through a clear and concise description of roles and responsibilities To spell out accountability
