Figure 1 Biceps tenodesis steps. (A) Through the cannula placed in the lateral portal, the tip of the hook-like instrument with a No. 2 braided suture was passed through the biceps tendon. A grasper introduced through the posterior portal was used to grab and pull the suture. The grasper held 1 side of the suture out of the lumen. (B) The opposite side of the suture still remained in the lumen. A suture retriever that entered through the anterior portal was passed under the tendon. (C) The tip of the suturing device was flipped over the opened jaw of the retriever to make it easy to grab the suture. (D) The grabbed suture was pulled out in the anterior portal. While the end of the suture was being held in the posterior portal, the retriever could pull the rest of the suture from the lumen. (E) Both ends of the suture were pulled out through the lateral portal cannula. (F) After decortication and creation of a hole for an anchor, the eyelet of the Bio-PushLock anchor and sutures were inserted into the bone hole with a hammer. (LHB, long head of biceps tendon.) Figure 3 Arthroscopic photo of right shoulder showing biceps tenodesis at bicipital groove. The arthroscope was located at the posterolateral portal. The intra-articular portion of the long head of the biceps tendon (LHB) was excised.
