COLORADO ACCESS SOLUTION: Virtually Integrated Behavioral Health Care Colorado Access launched the Virtual Care Collaboration and Integration (VCCI) Program in July 2017 Launched through AccessCare Services, the Telehealth and virtual care delivery arm of Colorado Access Virtual, integrated, team-based care to meet behavioral health care needs in primary care setting VCCI is available at no cost to all Colorado Access RAE PCMPs in Regions 3 & 5

COLORADO ACCESS SOLUTION: Virtually Integrated Behavioral Health Care Direct access to Psychiatrists and Behavioral Health Clinicians (BHC’s) via: Collaborative Consultations As On-demand as possible phone consultation from anyone involved with patient’s care in PCMP office to a psychiatrist or BHC Not limited to Medicaid, these consults are insurance- blind, funded by Colorado Access No limit, can call for consults as often as needed Direct Encounters (Telehealth) Psychiatrists and BHC’s provide direct care to members in the primary care setting using Telehealth Brief Intervention- typically 6 sessions or less Insurance-blind means the consults can be for any payer type, including all commercial insurances, Medicare, and patients with no insurance

COLORADO ACCESS SOLUTION: Virtually Integrated Behavioral Health Care VCCI Services include: Psychiatric evaluations Diagnostic assessments Medication Management Short-term counseling Bridging, and prescribing when necessary Workflow design and Telehealth implementation support Care Collaboration and Coordination with Colorado Access Care Management Provider education and training (Virtual Lunch and Learns with topics chosen by the practices) Care Collaboration and Coordination with Colorado Access Care Management- Can work with COA CM to connect members with long term care if needed and also refer to CM to connect patients with resources like food, clothing, shelter, etc
